Codex Health is actively looking for a full time Registered Nurse (RN) or Registered Dietitian (RD), Certified Diabetes Care & Education Specialist (CDCES) to join our talented team. As a RN/RD CDCES, you will play a pivotal role in supporting patients with cardiometabolic conditions including diabetes, hypertension, and obesity. This role reports directly to the Medical Director.

What you will do
In this role, you will collaborate with a cross-functional team including clinicians, health coaches, product managers, engineers, data scientists, designers and customer success professionals.
- Provide digital patient-centric care & evidence-based treatment plans for patients enrolled in the Codex Health Cardiometabolic Care Program
- Deliver lifestyle and behavioral support via microlearning strategies through video visits and app based learning modules
- Review remote patient monitoring data (scale, BGM, CGM, BP monitor)
- Provide feedback on areas for program improvement and expansion
- Develop and refine standardized documentation, workflows, and protocols
- Support the process for hiring, training and managing a staff of health coaches/clinicians to provide direct patient care
- Collaborate with the product and design team to ensure that clinical best practices are effectively translated into user-friendly features in an app and dashboard

Additionally, the team holds regular sessions to review and discuss major publications and clinical guidelines, and you will always be able to consult the team and medical director for questions of a clinical nature
What we’d like you to bring to the table
- RN or RD credential
- 5+ years of experience providing care for patients with obesity, diabetes, hypertension and dyslipidemia
- Certification as a certified diabetes care and education specialist (CDCES), with experience addressing wide-ranging topics in diabetes such as insulin self-management and diabetes distress
- Experience with continuous glucose monitoring is a big plus
- A patient-first mindset with an empathetic, judgment-free communication style
- A growth mindset with a passion for continually expanding your skills and knowledge for managing overall cardiometabolic health
- A high personal standard for evidence-based practices, with the humility to tell patients that you don’t know something but will find the answer to their question
- Experience with and/or a desire to work in a fast paced health technology startup environment
- Experience working with cross-functional team of engineers, product managers and designers to provide a clinical voice for product development is a plus
- Excellent problem-solving, organizational, and analytical skills
- High level of familiarity and comfort with technology such as Google Workspace (GCalendar, GSheets, GDocs, etc.), Slack and other basic software applications
Licensure Requirements
- Registered Nurse candidates: Must hold an active compact RN license (NLC)
- Registered Dietitian candidates: Active licensure in Pennsylvania, Illinois and Texas is strongly preferred. Licensure in Alabama, Delaware, Nevada, and New York is also preferred.
