SummaryThe PACT LPN at the Allentown Community Based Outpatient Clinic (CBOC) serves as one of the core members of the Patient Aligned Care Teams (PACT). The LPN works in collaboration with the Primary care Provider (PCP) - Registered Nurse Care Manager and the Medical Support Assistant (MSA).
